I too have put a lot of thought into "placebo effect" over many years. My conclusion is, to many, simply shocking: placebo effects are entirely caused by the doctor. I offer the Webster's dictionary definition, with a bit of writing between the lines, as proof.
Webster: placebo effect
noun
: improvement in the condition of a patient that occurs in response to treatment but cannot be considered due to the specific treatment used.
Explanation (filling in the blanks) by
Webster: placebo effect
noun
1. improvement in the condition of a patient (observed by a doctor)
2. that occurs in response to treatment (administered or observed by the doctor)
3. (For which the cause is not known to the doctor - if the cause is known, it's a real effect of the cause, not a placebo effect)
but cannot be considered due to the specific treatment used (in the mind of the doctor).
If we remove the doctor and the doctor's opinion, we are left with a real improvement with an unknown cause. Every real improvement has a real cause.

And I believe that will probably happen now that the neurotoxicity has gone. It’s all about timing. I see the path of life ahead of me as a gently flowing river with our destiny at the end. If we try and speed up our path along the river we may miss opportunities as our own efforts mean we are no longer at the right place at the right time. Similarly if we try slow things down, the opportunity we have been waiting for may have got sick of waiting and left already. Destiny is a celestial clock with opportunity placed at certain waypoints along the river. If we go with the flow of destiny life is easy. Everything we need is there for us, right when we need it. Fate, however is the actions of man thinking he can take short cuts. Fate interferes with cause and effect and puts us out of sync with the flow of the river of cause and effect. It takes a lot of effort too.
As you can see, I’ve been writing a lot lately. There was no point even trying a year ago. I would be no further ahead than I am now and would have suffered stress just trying.
I can see that I will make a difference with my writing someday soon. The readership has certainly shot up lately with a lot of new subscribers. While life has been a struggle and not particularly kind, the trade off is what I have learnt. Only someone with a life like mine gets the opportunity to learn what I’ve learnt. My words come from 100% experience which is a rarity in this day and age, and in my experience it’s the only reliable way to learn. Many are so far from thought invention, because they’ve been taught to learn a certain way.
I’ll certainly make a big impact with my writing in New Zealand. The size of the country makes it achievable. There is too much logic in what I write for it to be ignored, which also makes it dangerous for the status quo

Exactly. Effects don’t happen without causes.
